Full Circle Healing Center utilizes evidenced-based screening, assessment and evaluation instruments to identify both substance use and mental health needs.

What is the purpose of an evaluation?

• Identify presenting problems/need for treatment
• Establish or confirm a diagnosis
• Provide basis for medical / clinical necessity
• Establish an individual’s treatment plan
• Make recommendations for treatment
• Determine the appropriate level of care
• Initiate referrals to other treatment providers or identify resources for support

All new clients are evaluated prior to starting any service at FCHC.
